After being summoned for jury service: To check the status of your reporting time and date, you will need your 6 digit juror ID number and your 5 digit electronic signature number which are located on the front of your summons. After you have entered your personal individual numbers, the login page will also ask you to enter your date of birth.

After being summoned for jury service you MUST COMPLETE THE JUROR INFORMATION QUESTIONNAIRE WITHIN TEN (10) DAYS. When you log in to view your appearance date and time, you must click on the Complete Juror Information Questionnaire button. This will allow you to complete your Questionnaire electronically.
